There are a few places that you can send them to for reprogramming (do a google search for odometer reprogramming). It turns out that you actually have to remove the EEPROM where the data is stored, reprogram it, and solder it back onto the board.
I have monkeyed around with it a bit with some of my spare clusters. I was hoping to find that I could clip a piggy back socket on the chip and reprogram it in place, but unfortunately that doesn't work.

$350 Ouch! The process is actually fairly easy (desolder the 8 pin EEPROM, run the mileage through a magic program that generates the checksum to go along with that value, reprogram the EEPROM with the modified data, and solder it back down).
It is kind of a shady thing though, as theoretically here in the US it is illegal to modify a cars odometer. If the odometer is modified or replaced it must be documented (and that info even appears in a carfax on the car).

No. They can't be switched from KM to Mi.
