2001 5-speed Yaris Transmission Question Please

Hello to all, I'm hoping to get some information, I have a 2001 5-speed Yaris Hatchback 2-door, that is possibly having transmission problems, the Yaris goes shifts into gear's easily when turned off however, when I try to drive it, it jumps all out of gears, it will go into reverse, and forward but, kinda like bottoms out (clang) when I try to shift it from 3rd to 4th gear..

I was told by mechanically inclined friend, that the transmission need's to rebuilt..Can someone please tell me the model number for transmission rebuild kit?
