D-4D P252F + P2463 post injection issues (DPF)

Hello,

I'm running a 2011 Yaris D-4D, 220k km.

I'm having some constant issues with the DPF regeneration: it seems to be filling up quite fast, even in extraurban driving. It clogs and then when the regeneration kicks in, the sooth is cleaned way down to 15%.

Besides this, sometimes, I end up with the yellow wrench.
By scanning the OBD2 with Toyota Techstream, I have read the following codes:
P252F (excessive post injection fuel) and P2463 (DPF regeneration unusuccesful attempt).

These seem like recurrent issues.

I am hoping that other 1NDTV engines have faced this issue in the past.
Any tips?
