Your part list is correct. You don't need the 99-8216, just the 95-8202. You reuse the OEM radio's mounting brackets to mount the new HU.

OK, so I need:

Silver Toyota Dash Trim p/n 55405-52640
Metra Gap Filler 95-8202

I thought that I would need a Toyota to Pioneer wiring adapter so that I don't have to splice up the Toyota plug into the Pioneer. Any ideas on what that part number might be and if I need it? Are there any other parts that you guys can think of that I'll need?

Thanks for the help.

I used the same Metra kit to install my double-din radio. I also went to crappy tire (Canadian Tire, pep-boys or something similar in the US will probably carry them) and got the Toyota wiring harness kit and soldered it together with the Pioneer one.

Pic of the stereo installed (this is an '05 Echo hatch):

Any chance you know what the part number is for that wiring kit?

I'd also like to do this.

AVH-P4100DVD Bypass

I think I'll also need a Auto Zone #19273 Duralast Relay or a similar relay in order to bypass the safety features for full iPod control while I'm driving.

I started watching the video and was thinking why the heck isn't he just grounding the ebrake wire. Then, at the very end he mentions the need for that HU to see the ebrake input toggle, instead of just being permanently grounded.
