You mentioned that the mechanic had to top the fluid up. So assuming it was down and you'd lost fluid somewhere. There should be a small ventilation/inspection port on the front side of the bell housing. I'd recommend getting a camera probe in there to inspect the concentric slave to see if it's leaking.
