The exhaust is almost flush with the bottom of the back bumper or in my S's case the bottom ground effect. Mine does not stick out past the bumper at all and and there is really good clearance betweeen the top of the tip and and the bottom of the ground effect.

My TRD axle-back is the new (Borla) design. It sounds fine in the sedan. A bit louder past 3000 rpm but not uncomfortable. Actually the sound appears to be coming from the front, not the back (I have stock airbox).
It must be fairly mellow from the outside as it seldom turns pedestrians heads (no ricey fart-can) and my neighbours haven't commented on my daily 6 a.m. office departures. It does wake up wifey though.

Note: only the tip is visible with the S / Aero rear valance in place. Can't speak for standard body models. I've never seen one TRD equipped.
