Very unlikely that machining is your issue. Once you had the studs in, did you torque on an old lug nut to seat them in tight?

Also, I have the same spacers and found that Ichiba ships with extended studs that are way too long. This makes it extremely difficult to get the studs into the mounting plate. The studs only need to be 10mm longer than OEM, and the ones they ship are more like ~20mm longer. I had to get more appropriate length studs from Napa.
