Actually my 1960's Gravely tractor has a transmission like that. In the Gravely it's done with planetary gears. It can go from full speed forward through neutral to full speed reverse, often resulting in yikes moments. It behaves similarly to hydrostatic transmissions, but is entirely gear based.
Here's a cutaway of a Gravely:  Here's a picture of one like mine, with the tiller attachment on it. I actually used mine today to rototill my garden.
